Step 3 is after you are ECFMG certified i.e step 1, ck and cs cleared, documents verified.

You can take step 3 any time, before the end of year 1 of residency, provided you are on a J1 visa or are a permanent resident of US.

If you want to apply on an H1 visa, you need to clear step 3 before joining a residency program.

Time of residency depends on the specialty. Minimum is 4 years.
